PCI DSS Report Templates

Does anyone have any examples of the reporting templates used to report their PCI status to their merchant bank? I'm looking specifically for examples of the formats and general headings used within reports.

Please note I have the standard PCI checklists and spreadsheets which will be appendixed to the report.

Simplly Use the Word format of the Security Audit Procedures, from the PCI SSC website.
If the "field to field" restrictions bother you, then this may wwork for you:
1 save the document in RTF format
2. open the file in wordpad.
3. delete t to tools the handful of characters that follow 'password' (from memory - search for "passw")
4. save the file, and re-open in Word
5. Go to Tools, and 'unprotect'
6. Re-arrange to your hearts content

i was aware of that but wondering if there was anything else or is that as simple as it gets
